Annual membership for direct primary care, including on-call telephone triage and evaluation. Office, telemedicine or home visits for $200/month or $2200/year (4/month or as allowed by physician)

Includes lab orders, radiology orders, referrals as needed and a written summary of recommendations.

No additional fee for each office/telemed visit or annual physical.

Visits to you at the hospital available at doctors discretion for small additional fee.

For all Annual membership Direct Primary Care patients who have a child under the age of 26, they can add their children and receive the same benefits for only $50 per month.

For all Complete Direct Primary Care patients who have a spouse, they can add their spouse and receive the same benefits for only $100 per month.

For patients WITHOUT a membership, a one-time visit for physical exam at the office or home ($1200, within 15 mile radius from office address).

Includes lab order, radiology order, referrals as needed, written summary of recommendations, as well as video/telephone review of findings for no additional fee.

For patients WITHOUT a membership, evaluation of urgent condition with triage to hospital if appropriate. Visit at office or via telemedicine. $250. Visit to your home available for $350.

For patients WITHOUT a membership, an evaluation and management of on-going chronic conditions for $250 at office, via telehealth or telephone.

Visit to your home included at doctors discretion, or at your request for additional $100. Must be within 15 mile radius of Westlake Village Blvd. and Triunfo Canyon, 91361. Outside of this area for additional fee and at Doctors discretion

Includes prescription renewal, lab and radiology orders, referrals as necessary. Includes any necessary follow up telephone or telehealth for review of abnormalities found on testing.

For patients WITHOUT a membership, evaluation and Consultation regarding issues surrounding the use of addictive substances. Including alcohol, tobacco, prescription medications, marijuana, and other drugs of abuse at office, $1200.

Includes evaluation for level of care needed, and if appropriate for outpatient management, prescription, laboratory and referral for psychiatric care.

If not appropriate for outpatient treatment, a detailed list of options for referral for a higher level of care will be given (psychiatrist, intensive outpatient, sober living, residential or inpatient hospitalization, methadone clinic).

Medication assisted treatment, including naltrexone, suboxone/subutex, buprenorphine for select patients who qualify. Not all patients will qualify, and this is at the doctors discretion. Monthly MAT in office or via telehealth visits $250.

For patients WITHOUT a membership, at the office or via telemedicine if appropriate. Hospital Discharge follow-up; Admission to Assisted Living Facility/SNF; Post-surgical Evaluation all available at the rate of $350/visit. Visit to home available for $450.

Must be within 15 mile radius of Westlake Village Blvd. and Triuno Canyon Blvd., 91361. Visits outside this distance available for additional fee and at Doctors discretion.
